Position Summary• Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.

Education• Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience.

Licenses and Certification• Illinois license in Physical Therapy• BLS

Experience and Skills• Technical Experience: N/A

Role Specific Responsibilities• Performs thorough evaluation• Develops appropriate Plan of Care• Implements Plan of Care• Documentation is accurate, complete and timely• Provides supervision to assistants, rehab techs, and students
