Description
Pay Rate: $40 per hour to $51 per hour Shift: 8AM-4PM
Responsibilities:
Perform initial examinations, establish appropriate goals, plan and implement treatment interventions, perform timely re-examinations and modify goals and treatment interventions accordingly.
Coordinate with other disciplines when indicated.
Assume responsibility for all documentation requirements in a timely manner, including initial examinations, progress notes, monthly summaries, and discharge summaries.
Provide training to families or care partners and medical personnel as indicated.
Recommend appropriate durable medical equipment in coordination with appropriate team members and write letters of medical necessity when indicated.
Participate in discharge planning, make recommendations for continued physical therapy, and provide home exercise programs when appropriate.
Attend all appropriate conferences, rounds, in-services, and meetings.
Be assigned additional patients in the event that coverage is necessary as dictated by staffing needs.
Attend clinics as assigned.
Maintain therapy attendance records, patient charges, and statistics.
Participate in the supervision of Rehab Patient Care Associates, volunteers, and students.
Follow all company policies and procedures.
Requirements:
0 - 1 year of experience
Current New York State Physical Therapy license
Must be a current member of the APTA
Preferred Skills:
Excellent oral, communication, and interpersonal skills
