Job Title: Physical Therapist (PT)

Position Summary

The Physical Therapist provides professional therapy services to patients across all age groups, developing and implementing individualized treatment plans to improve mobility, function, and overall quality of life. This role focuses on evidence-based care, patient-centered treatment, care coordination, and collaboration with healthcare teams to achieve optimal outcomes.

Responsibilities

• Provide skilled physical therapy services to patients of all ages
• Evaluate patient needs and develop age-appropriate treatment plans based on clinical findings
• Plan, implement, and manage individualized physical therapy care plans
• Reassess patient progress and update treatment plans and physician orders as needed
• Apply evidence-based physical therapy principles and practices to support patient recovery
• Communicate effectively with patients, physicians, healthcare teams, managers, and external partners
• Utilize problem-solving and quality improvement strategies to enhance patient care and clinical processes
• Educate patients and caregivers on treatment plans, exercises, and strategies to improve functional outcomes
• Orient, train, and supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines and organizational policies

Qualifications

• Active Physical Therapist (PT) license in the applicable state required
• Graduate of an accredited Physical Therapy program
• Strong clinical assessment, treatment planning, and documentation skills
• Knowledge of current physical therapy practices and patient care standards
• Excellent communication and interpersonal skills
• Ability to work collaboratively within a multidisciplinary healthcare team
• Experience supervising or mentoring PTAs preferred
