Physical Therapist (Home Health) 

Schedule: Monday–Friday, 8:00 AM–4:30 PM 

Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last 

Coverage Areas: Cumberland, Hoke and Robeson

Dress Code: Royal Blue scrubs required

About the Role

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our Home Health team. This role is ideal for clinicians who value autonomy, one‑on‑one patient care, and the opportunity to make a meaningful impact in the home setting.

Key Responsibilities

• Evaluate patients’ functional status and physical therapy needs.

• Collaborate with the physician to develop and update individualized therapy plans of care.

• Deliver physical therapy services within state-defined scope of practice and in alignment with the established plan of care.

• Coordinate care with nurses, physicians, and other members of the healthcare team.

• Observe, document, and report patient responses to treatment and any changes in condition.

• Provide therapeutic interventions to relieve pain, restore function, and maximize patient independence using accepted standards of practice.

• Educate patients, families, and caregivers on the plan of care, functional limitations, and progress toward goals.

• Complete patient visits as outlined in the Plan of Treatment and submit timely, accurate progress notes per agency policy.

• Conduct comprehensive evaluations and develop PT plans of care within state-specific guidelines.

• Report evaluation outcomes, goals, and projected frequency of care.

• Deliver care effectively in diverse home environments and with patients of all ages and backgrounds.

Required Qualifications

• Current Physical Therapy license in the state of practice.

• Current CPR certification.

• Valid driver’s license, vehicle insurance, and reliable transportation (or access to public transit).

Preferred Experience & Skills

• Previous Home Health experience.
