The Home Health Physical Therapist will provide skilled physical therapy services in accordance with each patient’s individualized plan of care, including initial and ongoing assessment of progress and response to treatment. Accurately document evaluations, interventions, and patient outcomes while collaborating with the care team to support quality patient care. Consistently model Westminster-Canterbury's core values of Respect, Passion, and Professionalism and hold others accountable to the same standards.

Required
Education: Graduation from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or a CAPTE successor organization. Internationally educated candidates must have completed a program determined to be substantially equivalent to U.S. entry-level physical therapist education by an approved credentials evaluation organization.
OR
Examination: Successful completion of a state-approved physical therapist examination in the state where physical therapy services are provided.
At least one year of professional experience
