Pride Health is hiring a Physical Therapist Home Healthto support our clients in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
Job Title - Physical Therapist-Home Health

Job Type: Contract

Location- Tyler, TX

Shift:  Monday–Friday 8:00 am to 4:30 pm 

Hours- 40 hrs/ week 

Coverage Areas: Hawkins, Mineola, Lindale

Productivity Expectation: 30 points per week

Work Setting: On-site (clinic-based)

Mileage Reimbursement: Yes (between patient visits, from first to last visit)

Pay-$50 to $56/hr on W2

*Pay varies based on experience

Key Responsibilities:

  • Key Responsibilities
  • Perform comprehensive patient assessments in home health and hospice settings
  • Develop and implement individualized Plans of Care (POC)
  • Complete required documentation, including:
    • Start of Care (SOC)
    • Resumption of Care (ROC)
    • Recertifications and reassessments
  • Provide therapeutic interventions to improve patient function and independence
  • Supervise and collaborate with Physical Therapist Assistants (PTAs)
  • Educate patients and caregivers on treatment plans and home exercise programs
  • Ensure compliance with all clinical and regulatory standards

Required Qualifications:

  • Active Physical Therapist (PT) license
  • Prior experience in Home Health (HH)
  • Experience with Homecare Homebase (HCHB) documentation system
  • Strong clinical assessment and care planning skills
