What You'll Do- Provide skilled physical therapy services to patients in their homes
- Evaluate patient needs and develop individualized treatment plans
- Implement therapy techniques to promote mobility and function
- Educate patients and families on therapeutic exercises and device use
- Document patient progress accurately using EMR system (Meditech Home Care)
- Collaborate with healthcare team members to coordinate care
- Support support within the department as needed
What You Need- Valid Physical Therapy license in Louisiana
- 1+ year of experience in physical therapy, in settings such as acute care, home health, or rehabilitation
- Ability to work independently and communicate effectively with patients and caregivers
- Strong organizational skills and attention to detail
