Position Overview
We are seeking a compassionate and dedicated Physical
Therapist (PT) to provide high-quality physical therapy services to
patients in their residential homes throughout Pennsylvania.
The Physical Therapist will evaluate patients, develop
individualized treatment plans, provide appropriate therapeutic interventions,
monitor progress, and collaborate with patients, families, and other healthcare
professionals to support positive patient outcomes.
The ideal candidate is organized, dependable, comfortable
working independently, and committed to delivering patient-centered care.
Key Responsibilities
- Conduct
physical therapy evaluations in patients’ residential homes.
- Develop
individualized treatment plans based on patient needs, medical conditions,
and functional goals.
- Provide
appropriate physical therapy treatments and therapeutic interventions.
- Monitor
patient progress and modify treatment plans as needed.
- Educate
patients and caregivers on exercises, mobility, safety, and home programs.
- Communicate
effectively with physicians, nurses, patients, caregivers, and other
members of the healthcare team.
- Complete
accurate and timely clinical documentation and charting.
- Utilize
electronic medical record (EMR) systems to document patient care.
- Effectively
manage patient schedules and daily workload.
- Maintain
compliance with applicable clinical, regulatory, and documentation
requirements.
- Travel
locally to and from patients’ residential homes.
Required Qualifications
- Bachelor’s
degree or higher.
- Bachelor’s
or Master’s degree in Physical Therapy from an accredited college or
university preferred/required based on applicable licensing requirements.
- Minimum
of 1 year of healthcare experience.
- Minimum
of 1 year of experience using an Electronic Medical Record (EMR),
including Practice Partner or another EMR system.
- Current
and valid Physical Therapist license in the state of Pennsylvania (PA).
- Current
and valid regular driver’s license.
- Ability
to travel locally to patient residences, with approximately 20% of the
workday spent driving.
- Strong
organizational and time-management skills.
- Ability
to independently manage patient schedules and prioritize daily
responsibilities.
- Ability
to consistently meet required deadlines for clinical documentation and
charting.
- Demonstrated
experience using tablets, smartphones, and EMR software.
- Strong
communication, interpersonal, and patient-care skills.
License Verification
- PT
license number must be provided for license/name clearance.
Employment History Requirement
- Candidates
must not have worked for or applied to Residential within the past 6
months.
Schedule
- Monday–Friday:
8:00 AM–5:00 PM
- One
Saturday per month
Travel
- Local
travel to patients’ residential homes is required.
- Approximately
20% of the workday may involve driving to and from patient locations.
