Job Summary
Empath Home Health, a member of Empath Health, is seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team in Sarasota, FL.
As a Home Health Physical Therapist, you'll help patients regain and maintain their highest level of physical function while delivering personalized care in the comfort of their homes. You'll work alongside a collaborative clinical team dedicated to improving patients' quality of life through Empath Health's Full Life Care philosophy.
Key Responsibilities
- Provide skilled physical therapy services as prescribed by a physician.
- Perform comprehensive initial and ongoing patient assessments, including OASIS assessments.
- Evaluate patients' functional abilities and activities of daily living (ADLs).
- Develop, implement, and revise individualized treatment plans in collaboration with physicians and the care team.
- Monitor patient progress and adjust treatment plans as needed.
- Communicate changes in patient condition and treatment response to the physician and appropriate care team members.
- Ensure documentation is accurate, timely, and compliant with agency policies and state/federal regulations.
- Educate patients and caregivers on treatment plans, exercises, and strategies to improve mobility and independence.
Minimum Qualifications
- Doctor of Physical Therapy (DPT) from a CAPTE-accredited program
- Active Florida Physical Therapist License
- Current BLS Certification
(American Heart Association or American Red Cross) - One to two (1–2) years of Physical Therapist experience
