We are a respected integrative physical medicine clinic serving the Fort Wayne community. Our mission is to help patients achieve long-term healing and improved quality of life through personalized treatment plans and evidence-based care.
Our team includes experienced medical providers, rehab specialists, and support staff who work together to deliver comprehensive care for acute and chronic pain conditions.
Responsibilities

• Perform patient evaluations and physical therapy assessments
• Review medical history, imaging, and physician referrals
• Develop individualized treatment plans based on patient needs and goals
• Guide patients through therapeutic exercises, modalities, mobility training, and rehabilitation programs
• Educate patients on treatment plans, home exercises, posture, stretching, and lifestyle modifications
• Monitor patient progress and perform reassessments
• Maintain accurate and timely EMR documentation
• Collaborate with physicians and multidisciplinary providers to ensure optimal patient outcomes

Requirements

• Graduate of an accredited Physical Therapy program
• Active Indiana Physical Therapy license

Compensation & Benefits

• $90K–$105K Base Salary + Monthly Bonuses
