    Job Overview:

    A dedicated Physical Therapist is needed to join an interdisciplinary team (IDT) focused on providing high-quality, patient-centered care. This role is crucial in coordinating and delivering rehabilitative services to enhance participant independence and mobility, whether at the center or in-home settings.

    Key Responsibilities:

    • Conduct comprehensive Physical Therapy assessments in both facility and home environments.
    • Develop and implement customized care plans in collaboration with an interdisciplinary team.
    • Provide active and passive therapy interventions, including muscle re-education, gait training, and ADLs (Activities of Daily Living).
    • Utilize treatment methods such as massage, heat, sound, water, light, or electrical therapy.
    • Supervise Therapy Assistants, offering training and guidance for Functional Maintenance Programs (FMP).
    • Educate staff, caregivers, and family members on therapeutic equipment usage and patient mobility strategies.

    Qualifications:

    • Bachelor’s Degree in Physical Therapy with at least 5 years of experience.
    • Master’s or Doctorate in Physical Therapy (Preferred).
    • Valid California Physical Therapy License or eligibility to obtain one.
    • Minimum of 2 years of experience in a clinical setting, preferably working with frail or elderly populations.
    • CPR certification preferred.
    • If educated outside the U.S., the Physical Therapy degree must be evaluated for U.S. equivalency.

    Compensation & Benefits:

    • Base Salary: $100,000 - $120,000 (DOE)
