About the Opportunity
A leading rehabilitation services provider is seeking a Physical Therapist to join an established therapy team within a respected senior living and rehabilitation community.
This role provides the opportunity to evaluate patients, create treatment plans, supervise PTAs, and collaborate with a multidisciplinary care team focused on helping residents improve function, mobility, and quality of life.
What You'll Do
- Evaluate and assess patients
- Develop individualized treatment plans
- Implement evidence-based physical therapy interventions
- Monitor treatment effectiveness and progress
- Modify plans of care as appropriate
- Supervise Physical Therapist Assistants
- Collaborate with physicians and interdisciplinary teams
- Complete timely clinical documentation
- Participate in discharge planning
- Support quality and compliance initiatives
What You'll Need
- Bachelor's, Master's, or Doctorate Degree in Physical Therapy
- Active Indiana Physical Therapy License
- Strong communication skills
- Excellent organizational and time-management abilities
- Ability to supervise PTAs
- Commitment to patient-centered care
Preferred Qualifications
- Skilled Nursing experience
- Long-Term Care experience
- Senior Living experience
- Rehabilitation experience with geriatric populations
- Experience in interdisciplinary care settings
