    A leading rehabilitation services provider is seeking a Physical Therapist to join an established therapy team within a respected senior living and rehabilitation community.

    This role provides the opportunity to evaluate patients, create treatment plans, supervise PTAs, and collaborate with a multidisciplinary care team focused on helping residents improve function, mobility, and quality of life.

    What You'll Do

    • Evaluate and assess patients
    • Develop individualized treatment plans
    • Implement evidence-based physical therapy interventions
    • Monitor treatment effectiveness and progress
    • Modify plans of care as appropriate
    • Supervise Physical Therapist Assistants
    • Collaborate with physicians and interdisciplinary teams
    • Complete timely clinical documentation
    • Participate in discharge planning
    • Support quality and compliance initiatives

    What You'll Need

    • Bachelor's, Master's, or Doctorate Degree in Physical Therapy
    • Active Indiana Physical Therapy License
    • Strong communication skills
    • Excellent organizational and time-management abilities
    • Ability to supervise PTAs
    • Commitment to patient-centered care

    Preferred Qualifications

    • Skilled Nursing experience
    • Long-Term Care experience
    • Senior Living experience
    • Rehabilitation experience with geriatric populations
    • Experience in interdisciplinary care settings
