Physical Therapist (PT) Home Health
Location: 4259 W Swamp Rd, Doylestown, PA 18902
8:00am-4:30pm
Position Overview
We are seeking an experienced Physical Therapist (PT) to provide skilled care to patients in a home health setting. The ideal candidate will have strong home health experience and be comfortable performing both Start of Care (SOC) and revisit visits.
Requirements
- Active Pennsylvania PT license required at the time of submission.
- Minimum 2+ years of home health Physical Therapy experience.
- Start of Care (SOC) experience required.
- Revisit experience required.
- Strong clinical assessment, documentation, and patient-care skills.
- Ability to provide therapy services independently in patients' homes.
