Responsibilities include: \n\n Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy. \n \n\n Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants. \n \n\n Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients need are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines. \n \n\n Manages the activities of the multi-disciplinary team providing care to the patient. \n \n\n Monitors the provision of therapeutic services through written and verbal communication with all disciplines. \n \n\n Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team. \n \n\n Interprets agency policy to patients/families. \n \n\n Reviews patients visit weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits. \n \n\n Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor. \n \n\n \nHome Health Physical Therapist Requirements: \n\n Bachelor's Degree Physical Therapy \n \n\n Physical Therapist State of Florida License \n \n\n 1-year Physical Therapist experience \n \n\n Basic Life Support \n \n\n Driver's License (Florida driver's license is required within 30 days of hire) \n \n\n Preferred Master's Degree Physical Therapy \n \n\n \nThis position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA).
