    The Physical Therapist provides comprehensive physical therapy services by assessing patients' conditions, developing individualized plans of care, delivering appropriate therapeutic interventions, and evaluating patient progress.

    The Physical Therapist will perform initial and ongoing assessments, establish and revise plans of care, provide therapy interventions using standard physical therapy techniques, and collaborate with patients, families, physicians, physical therapy assistants, students, and other healthcare professionals.

    This position requires an experienced Physical Therapist with Skilled Nursing Facility (SNF) experience who is comfortable providing onsite patient care and working occasional holidays and one weekend per month.

    Key Responsibilities
    Perform initial and ongoing assessments of patients' physical and functional conditions.
    Establish, revise, and evaluate individualized plans of care based on identified patient needs.
    Involve patients and their families in the development and implementation of the plan of care.
    Perform physical therapy interventions utilizing standard physical therapy techniques and skills appropriate to the patient's condition.
    Monitor patient progress and modify treatment plans as clinically appropriate.
    Provide therapeutic services designed to improve patient mobility, strength, balance, function, and independence.
    Document patient assessments, treatment interventions, progress, and outcomes accurately and timely.
    Collaborate with physicians, nurses, physical therapy assistants, occupational therapists, speech therapists, and other members of the healthcare team.
    Oversee the activities of physical therapy assistants, students, and other support personnel as appropriate.
    Provide appropriate guidance, supervision, and direction to therapy assistants and students.
    Educate patients and families regarding therapy programs, exercises, mobility, safety, and continued care.
    Maintain compliance with established clinical, safety, infection prevention, and documentation standards.
    Provide compassionate, patient-centered care throughout the rehabilitation process.
    Participate in multidisciplinary care planning and communication as required.
    Work occasional holidays and one weekend per month as required.
    Perform other physical therapy duties as assigned.

    Required Qualifications
    Current and valid Physical Therapist license appropriate for the state of Oregon.
    Minimum 2 years of Physical Therapist experience.
    Recent Skilled Nursing Facility (SNF) experience required.
    Experience performing initial and ongoing patient assessments.
    Experience establishing, revising, and evaluating individualized plans of care.
    Experience providing physical therapy interventions using standard PT techniques.
    Strong clinical assessment, critical-thinking, and problem-solving skills.
    Ability to evaluate patient progress and modify treatment plans appropriately.
    Ability to work effectively with patients, families, physicians, therapists, and other healthcare professionals.
    Experience overseeing or working with physical therapy assistants, students, and support personnel.
    Strong communication and interpersonal skills.
    Ability to work the required Monday–Friday, 7:30 AM–6:00 PM schedule.
    Ability to work 40 hours per week.
    Willingness to work some holidays and one weekend per month.
    Ability to work onsite in Baker City, Oregon.

    Preferred Qualifications
    Recent traveler/contract Physical Therapist experience.
    Experience in Skilled Nursing Facility rehabilitation.
    Experience working with medically complex or post-acute patients.
    Experience collaborating within multidisciplinary rehabilitation teams.
    Strong patient and family education experience.
    Ability to adapt treatment approaches based on individual patient needs and functional goals.
