Position Summary
Fountain Care at Sunset Hills is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team. The PTA will work under the direction and supervision of a licensed Physical Therapist to provide high-quality therapy services designed to help residents improve mobility, strength, balance, independence, and overall quality of life.

Responsibilities
- Provide physical therapy treatments and interventions as directed by the supervising Physical Therapist.
- Assist patients with therapeutic exercises, functional mobility, transfers, gait training, balance activities, and other prescribed interventions.
- Monitor patient responses to treatment and communicate progress or concerns to the Physical Therapist and interdisciplinary team.
- Document treatments, patient progress, and other required clinical information accurately and timely.
- Follow individualized treatment plans and established clinical standards.
- Educate and encourage patients and caregivers regarding exercises, mobility, safety, and appropriate use of equipment.
- Promote patient safety and maintain a clean and organized therapy environment.
- Participate as appropriate in care planning and interdisciplinary communication.
- Maintain compliance with applicable federal, state, facility, and professional standards.
Qualifications
- Current Physical Therapist Assistant license in the State of Missouri.
- Graduate of an accredited Physical Therapist Assistant program.
- Current CPR/BLS certification, as required.
- Strong communication, organization, and interpersonal skills.
- Compassionate, dependable, and patient-focused approach.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Previous experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
