    Physical Therapist Assistant (PTA) – Per Diem

    Golden Crest Subacute Rehabilitation Center

    Location: 100 Smithfield Road, North Providence, RI 02904

    Golden Crest is seeking a compassionate and dedicated Per Diem Physical Therapist Assistant (PTA) to join our rehabilitation team in an in-house subacute rehabilitation setting. The PTA will work under the direction and supervision of a licensed Physical Therapist to provide skilled therapy services that help residents improve mobility, strength, balance, and overall functional independence.

    Responsibilities:

    • Provide skilled physical therapy treatments as directed by the supervising Physical Therapist
    • Assist residents with therapeutic exercises, mobility training, balance activities, and functional tasks
    • Monitor resident progress and communicate changes to the Physical Therapist and interdisciplinary team
    • Document treatment sessions accurately and in accordance with professional and facility standards
    • Promote resident safety, comfort, and participation throughout the rehabilitation process
    • Collaborate with nursing staff and other healthcare professionals to support individualized care plans

    Qualifications:

    • Current Physical Therapist Assistant license in the state of Rhode Island
    • Graduate of an accredited Physical Therapist Assistant program
    • Experience in skilled nursing, subacute rehabilitation, or similar healthcare settings preferred
    • Strong communication and teamwork skills
    • Commitment to providing compassionate, resident-centered care
