As a Home Health Physical Therapist Assistant (PTA) with Elara Caring, you’ll provide personalized therapy to patients throughout the Easton territory. Working under the direction of a licensed Physical Therapist, you’ll turn individualized treatment plans into meaningful, real-life progress.
One-on-one care: Give each patient your focused attention.
Monday–Friday schedule: Build greater consistency into your workweek.
Field-based independence: Manage your daily visits throughout a localized territory.
Meaningful outcomes: Help patients improve mobility, safety, and independence at home.
Team support: Partner with Physical Therapists and an experienced interdisciplinary care team.

As a Physical Therapist Assistant, you will:
Carry out individualized treatment plans established by the supervising Physical Therapist.
Help patients improve strength, balance, mobility, coordination, and functional independence.
Guide patients through therapeutic exercises and activities based on their personal goals.
Teach patients and caregivers how to safely continue prescribed exercises between visits.
Provide instruction on the proper use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices.
Monitor each patient’s progress and promptly report changes in condition or response to treatment.
Complete accurate and timely clinical documentation.
Follow physician orders, the established Plan of Care, state practice standards, and organizational policies.
Collaborate with the Physical Therapist and other care team members to support safe, successful outcomes.

Graduation from an accredited Physical Therapist Assistant program.
Current, unrestricted PTA license in the state of practice.
Six months of Physical Therapist Assistant experience preferred.
Home health experience is a plus.
Valid driver’s license, dependable transportation
