    Job Description

    What you will be doing:

    • Implement individualized physical therapy treatment plans developed by the Physical Therapist.
    • Assist with participant assessments, including vital signs, range of motion, strength testing, and functional mobility.
    • Deliver therapeutic interventions including therapeutic exercise, gait training, balance training, muscle re-education, and functional mobility training.
    • Assist participants in improving performance with activities of daily living (ADLs).
    • Coordinate participant treatment schedules, therapy visits, and related documentation.
    • Collaborate with physicians, therapists, nurses, and interdisciplinary team members to provide comprehensive care.
    • Maintain accurate clinical documentation in accordance with regulatory and organizational standards.
    • Provide therapy services in both outpatient center and home-based settings as needed.

    Experience you will need:

    • Associate's Degree from an accredited Physical Therapist Assistant (PTA) program.
    • Valid Physical Therapist Assistant (PTA) License or proof of license eligibility.
    • Knowledge and experience providing therapy services to frail or medically complex older adults.
    • 1 to 2 years of PTA experience preferred.
    • Experience in one or more of the following settings: Outpatient Rehabilitation, Acute Care, Inpatient Rehabilitation, Skilled Nursing Facility (SNF), Home Health.
    • Experience treating adult and/or geriatric populations.
    • Strong communication, teamwork, and patient education skills.
    • Candidates with only pediatric experience will not be considered.
