Elara Caring is seeking a compassionate Physical Therapist to serve patients throughout the New Haven territory. In this field-based role, you will provide personalized, one-on-one therapy while helping patients improve their safety, independence, and quality of life.

What You’ll Do

• Evaluate patients’ mobility, strength, safety, and functional needs
• Develop and implement individualized treatment plans
• Provide skilled physical therapy services in patients’ homes
• Help patients improve mobility and safely perform daily activities
• Educate patients and caregivers on exercises, equipment, and safety techniques
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians, caregivers, nurses, therapy professionals, and other members of the care team
• Communicate changes in patient condition to the appropriate clinical team members
• Complete accurate and timely clinical documentation
• Supervise therapy assistants and support staff, as appropriate
• Support patients in reaching their highest possible level of independence

What You’ll Bring

• Graduate of an American Physical Therapy Association-approved physical therapy program
• Current and unrestricted Connecticut Physical Therapist license
• Minimum of one year of experience in a healthcare facility
• Strong communication, organization, and patient-care skills
• Ability to work independently in a home-based environment
• Willingness to travel throughout the assigned Norwalk and Fairfield territory
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

The base salary for this position is $100,000 to $115,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
