Day Kimball Health is hiring a Physical Therapist to join our Homecare team in Thompson, CT!

Position: Physical Therapist
Department: Homecare – Physical Therapy
Schedule: Days | 8:30 AM – 4:30 PM

This position is eligible for a $12,000 sign on bonus for external candidates

Physical Therapist Job Summary:

Under the supervision of the HomeCare Rehabilitation Supervisor, organizes and administers prescribed physical therapy programs in an effort to reduce and/or prevent disability and to assist patients in achieving maximum functional potential.

Physical Therapist Job Responsibilities:

Evaluates and treats patients as prescribed by referring physician. Records treatment, response, progress and discharge information in patient’s electronic chart using portable electronic device. Educates patient, family and significant others regarding condition, treatment and home program. Routinely confers with physician and other practitioners in person, by phone, or in writing, regarding progress, regression or change of plan.
Orients, instructs, and directs other Health Care Team members as needed.
Evaluates and treats patients as prescribed by referring physician. Reviews physicians referral and patients condition and medical records to determine physical therapy treatment required.
Records treatment, response, progress and discharge information in patient’s electronic chart using portable electronic device. Tests and measures patient’s strength, motor development, sensory perception, functional capacity, and respiratory and circulatory efficiency, and record findings to develop/revise treatment program.
Plans and prepares written treatment plan based on evaluation of patient data with realistic, objective, long and short term goals and time frame.
Initiates/attends case conferences when necessary.

Physical Therapist Education and Experience Requirements:

B.S. Degree and graduate from an accredited school of physical therapy required.
Connecticut license or Connecticut license eligible required.
CPR Certification required.
Minimum of one year experience preferred.
Valid Connecticut Driver’s License and proof of current insurance coverage required.

Physical Therapist Knowledge Skills and Abilities:

Must be proficient in Reasoning, Math, Reading, Writing, and Speaking: Speak with Poise, voice control and confidence using correct English and well-modulated voice.
Data: Determines time, place, and sequence of operations or actions to be taken on the basis of analysis of data, executing determination of and/or reporting on events.
People: Follows instructions and procedures in fulfilling duties. May attend to the needs, requests of people. Immediate response is usually involved.
Things: Uses body members, hand tools, special devices to work, move, carry objects or materials. Involves little latitude for judgement with regard to attainment of standards.
Uses common sense and special medical skills to care for the sick or handicapped.
Understands technical information from supervisors, charts, reference books, manuals and labels.
Ability to use Portable Electronic Device to record/maintain patient information pertaining to Plan of Care.
Uses eyes, hands and fingers with skill.
Works fast and accurately in an emergency.
Communicates with people when they are sick, handicapped or nervous.
Changes from one duty to another frequently.
Identifies and acts on priorities for smooth departmental operations.
Maintains discretion in handling confidential nature of information available.
Keeps up to date on trends and developments in the field of physical medicine and health
