Summary:
The Department of Pharmacy is seeking a highly motivated, energetic, and dynamic individual to serve patients across the transitions of care continuum. The Pharmacy Transition Specialist working with the Central Outpatient Pharmacy team will be responsible for logistically organizing, filling, collaborating and/or delivering required discharge medications to patients and/or families or caregivers. The Specialist will ensure the successful hand-off to the appropriate medications to enable the improvement of the patient's health management and experience.
This position qualifies for our Pharmacy Technician Incentive Program, which includes $5000 in commitment incentives spread over a two-year period. Payment is made after six months, one year, and two years of employment.

Education Requirements:
Completion of a nine-month Pharmacy Technician Program (or equivalent combination of education, training and experience).
Licensure/Certification Requirements:
Registration with the NC Board of Pharmacy unless enrolled in a PharmD program.·CPhT from PTCB or ExCPT from NHA unless enrolled in a PharmD program.
Professional Experience Requirements:
If completion of a Pharmacy Technician Program: No prior experience required.
If a High School diploma or GED: Two (2) years of experience as a Pharmacy Technician.
