General Summary of Position
Evaluates drugs prescribed by physicians for appropriateness of dose therapeutic efficacy allergy drug interaction and contraindications; processes prescriptions through insurance company and handles DUR rejects pre-authorizations etc.; performs final check on all prescriptions ensuring accurate input labeling and dispensing; and provides counseling and drug information to patients and physicians. Primary Duties and Responsibilities
Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.Acts as the pharmacist in charge in the absence of the manager: directing workflow patterns ensuring daily responsibilities are carried out by assigned staff resolving issues requiring management intervention and performing routine oversight of pharmacy operations.Performs final check on all completed prescriptions verifying the accuracy of input labeling and dispensing; makes corrections as necessary; maintains refill and narcotic logs and prescription files.Processes prescriptions in the dispensing system; evaluates prescriptions for appropriateness of dose therapeutic efficacy allergy drug interactions and contraindications; handles DURs rejects preauthorizations etc.; communicates with third party payors to troubleshoot issues relating to coverage reimbursement etc.Provides clinical information and guidance to physicians and allied health professionals as appropriate to provide safe prompt and accurate dispensing of all prescriptions; calls physicians and follows up on refill requests; notifies physicians of Rx's requiring pre-authorizations and discusses alternatives as appropriate.Provides patient counseling as appropriate on a patient-by-patient basis.Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
