A growing Long-Term Care (LTC)–focused organization in Monsey is seeking a Licensed Pharmacist to support partner facilities with clinical pharmacy management and medication optimization. This role plays a key part in ensuring safe, effective, and compliant medication use across long-term care settings while collaborating closely with healthcare providers to improve patient outcomes.
The ideal candidate has hands-on LTC pharmacy experience, strong clinical judgment, and the ability to work both independently and collaboratively in a fast-paced healthcare environment.
Key Responsibilities:
- Manage Prior Authorizations, Formulary Exceptions, Step Therapy, and Quality Care Dosing initiatives
- Provide clinical guidance and recommendations on formulary and non-formulary medications
- Collaborate with physicians, nurses, and facility staff to optimize medication regimens and improve patient outcomes
- Conduct comprehensive chart reviews and document clinical interventions
- Support and participate in quality improvement initiatives across partner facilities
- Ensure compliance with LTC pharmacy regulations, best practices, and clinical guidelines
- Assist facilities with medication-related problem-solving and clinical decision-making
- Contribute to antimicrobial stewardship efforts and medication safety programs
Qualifications:
- Active Pharmacist license (required)
- Previous Long-Term Care (LTC) pharmacy experience (required)
- Strong knowledge of therapeutic interchanges, formulary management, and LTC medication protocols
- Excellent communication, documentation, and organizational skills
- Ability to work independently while collaborating effectively with interdisciplinary teams
- Strong attention to detail and clinical accuracy
- Antimicrobial Stewardship experience is a plus
