The Permanent Supportive Housing Case Manager works with individuals and families who have experienced chronic homelessness and are living with disabling conditions. You'll help clients navigate the housing process from eligibility and housing search all the way through move-in and long-term housing stability.This position follows a Housing First approach, meaning we believe everyone deserves a safe and stable place to call home. You'll build relationships with clients, understand their individual goals and needs, and provide the ongoing support they need to maintain housing and build greater stability and independence.You'll also work closely with landlords, Housing Connect, community providers, and First Step House teams to address barriers and resolve issues before they put a client's housing at risk.
