Responsibilities

• Assess patients and develop individualized treatment plans under supervision.

• Implement therapeutic interventions and track patient progress weekly.

• Document patient status, treatment plans, and outcomes in EHR system.

• Collaborate with a multidisciplinary team to coordinate care.

• Monitor and report measurable outcomes to drive target-focused improvements.

• Educate patients and families on home programs and self-management.

• Assist in intake assessments and initial screenings for new patients.

• Maintain equipment and ensure safety and infection control standards.

• Support the implementation of evidence-based practices in therapy.

• Assist in developing and refining patient care pipelines and workflows.

• Participate in professional development and pursue relevant certifications.

Experience/Qualifications

• Licensed Physical Therapist Assistant in good standing with applicable state license.

• Ability to assess patients and develop individualized treatment plans under supervision.

• Proficiency in documenting care plans and outcomes in the EHR system.

• Experience or willingness to track weekly patient progress and adjust plans.

• Strong collaboration with multidisciplinary teams to coordinate patient care.

• Knowledge of infection control, safety standards, and equipment maintenance.

• Commitment to evidence-based practice and continuous quality improvement.

• Ability to educate patients and families on home programs and self-management.

• Participation in intake assessments and initial screenings for new patients.

• Willingness to pursue professional development and relevant certifications.
