What Youll Do- Coordinate and develop patient care plans focusing on quality and medical necessity
- Assess patient needs and evaluate levels of care appropriate for each individual
- Implement, monitor, and adjust care strategies to ensure positive health outcomes
- Collaborate with families, healthcare teams, and external providers
- Ensure timely delivery of services and compliance with healthcare regulations
- Maintain professional and compassionate interactions with patients and team members
What You Need- Active Kansas healthcare license (LPN, RN, or applicable license)
- Bachelor of Social Work (BSW) certification for Social Worker role
- Strong communication and organizational skills
- Ability to work collaboratively in a team environment
- Experience in acute or community healthcare settings preferred but not required
