What You’ll Do:
- Provide outpatient care for geriatric patients with chronic and complex health conditions
- Manage preventive care and chronic disease treatment plans
- Conduct geriatric evaluations and assessments
- Collaborate with a multidisciplinary team to optimize patient outcomes
- Document patient visits using EMR systems, primarily Epic
- Participate in call coverage (approximately one week per year), including after-hours and weekends
- Offer patient education focused on healthy aging and disease management
What You Need:
- Valid license/certification in Texas or eligible to obtain
- 1+ year of outpatient geriatric management experience
- Strong clinical assessment and documentation skills
- Experience managing chronic and complex conditions in an outpatient setting
