Per Diem Hospice Spiritual Counselor / Chaplain

Position Summary


Topkare Hospice, Inc. is seeking a compassionate and dependable Per Diem Hospice Spiritual Counselor / Chaplain to provide spiritual and emotional support to hospice patients and their families/caregivers.


The Spiritual Counselor/Chaplain serves as an important member of the hospice interdisciplinary team, providing individualized spiritual care that respects each patient's personal beliefs, values, culture, and preferences. This position provides spiritual care directly and may also coordinate services with community clergy and other spiritual care providers.


Key Responsibilities


  • Complete spiritual assessments of hospice patients and families/caregivers and ensure appropriate spiritual care services are provided in a timely manner.
  • Participate in comprehensive assessments and assist in developing individualized plans of care based on identified spiritual needs.
  • Provide direct spiritual care, counseling, presence, and support to patients and families/caregivers.
  • Respect and support patients and families from diverse religious, spiritual, cultural, and personal backgrounds.
  • Serve as a liaison and resource for community chaplains, clergy, congregations, and spiritual counselors.
  • Document spiritual care visits, services provided, and ongoing communication with community spiritual care providers.
  • Participate in patient care conferences and interdisciplinary group meetings as required.
  • Provide spiritual care consultation, education, and support to members of the interdisciplinary hospice team.
  • Assist with developing community relationships and spiritual care resources to meet patient and family needs.
  • Participate in quality assessment and performance improvement activities.
  • Provide or assist with funeral and memorial services when requested.
  • Participate in periodic memorial services for families, hospice personnel, volunteers, and community spiritual care partners.
  • Assist with the supervision and support of spiritual care volunteers when assigned.
  • Perform other related duties and responsibilities as needed.
Qualifications


  • Bachelor's degree or equivalent experience working with individuals and families experiencing death, dying, grief, or end-of-life care.
  • Ability to work collaboratively as part of an interdisciplinary hospice team.
  • Comfortable providing support in an interfaith and spiritually diverse environment.
  • Ability to respect and support different lifestyles, cultures, beliefs, and values.
  • Ability to establish and maintain relationships with community clergy and congregations.
  • Knowledge of and commitment to the hospice philosophy of care.
  • Valid driver's license and access to a reliable, properly insured vehicle in good working order.
  • Previous hospice experience preferred.
Schedule


This is a per diem/PRN position, with hours and patient visits scheduled based on hospice census and patient/family spiritual care needs.
