Responsibilities:- Conduct skilled nursing assessments and develop individualized patient care plans
- Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
- Monitor and evaluate patient health status, reporting changes to the physician as needed
- Educate patients and family members on disease management, medications, and care techniques
- Collaborate with interdisciplinary care teams including therapists, social workers, and physicians
- Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
- Ensure patient safety and comfort while promoting independence and quality of life
Requirements:- Active California Registered Nurse (RN) license in good standing
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Current CPR/BLS certification required
- Valid California driver's license, reliable transportation, and auto insurance
- Familiarity with OASIS documentation and Medicare home health guidelines is a plus
- Strong clinical assessment, communication, and time management skills
- Compassionate, self-motivated, and able to work independently in a field-based environment
