We are seeking a Pharmacist to join a hospital-based healthcare team providing care to patients with complex medical needs. This role offers a highly collaborative environment where pharmacists work closely with physicians, nurses, patients, and families to support medication management and optimize patient outcomes.
Schedule
Key Responsibilities
- Perform medication reconciliation and assist with discharge medication planning.
- Provide medication education and counseling to patients and caregivers.
- Collaborate with the interdisciplinary care team regarding medication therapy recommendations.
- Participate in antimicrobial stewardship activities, including review of culture results and therapeutic recommendations.
- Monitor and manage pharmacokinetic dosing and anticoagulation therapy.
- Evaluate medication regimens for appropriateness, interactions, adverse effects, and therapeutic effectiveness.
- Contribute to medication safety, quality improvement, and regulatory compliance initiatives.
Preferred Qualifications
- Hospital or inpatient pharmacy experience preferred.
- Experience with antimicrobial stewardship programs.
- Pharmacokinetic dosing and anticoagulation management experience preferred.
- Electronic medical record experience preferred.
- Strong communication skills with the ability to work effectively with patients, families, and healthcare professionals.
- Comfortable caring for patients with a broad range of medical conditions and medication therapies.
