What You’ll Do

• Conduct OT assessments for ADLs, IADLs, and safety
  
  
• Develop individualized care plans with measurable goals
  
  
• Provide training in adaptive strategies, energy conservation, and home modifications
  
  
• Educate patients/caregivers on safety and independence techniques
  
  
• Document per NJ DOH, Medicare, and BrightStar standards
  
  
• Collaborate with the care team to maximize patient outcomes
  
  

What We Need

• NJ Occupational Therapy license in good standing
  
  
• 1+ year OT experience (home health preferred)
  
  
• CPR/BLS certification
  
  
• Reliable transportation in Camden/Cherry Hill
  
  
• Strong clinical reasoning and patient education skills
