Pediatric Social Worker II (LCSW) (Per Diem)

Position Summary

The Pediatric Social Worker II provides psychosocial support, care coordination, advocacy, and community resource navigation for pediatric patients and their families enrolled in Navian Hawaii's Pediatric Palliative and Hospice Care Program. This position works closely with children facing serious illness and their caregivers to support emotional well-being, adjustment to illness, care planning, and access to resources. The Pediatric Social Worker II also provides guidance and support to siblings and family members, helping families navigate complex medical, emotional, financial, and end-of-life challenges while promoting quality of life and family-centered care.

Primary Responsibilities

• Conduct comprehensive psychosocial assessments of pediatric patients and their families to identify emotional, social, cultural, environmental, and financial needs.

• Provide counseling, emotional support, crisis intervention, and anticipatory grief support to children, caregivers, and siblings throughout the course of illness.

• Collaborate with the pediatric interdisciplinary team to develop and implement family-centered plans of care that support the unique needs of each child and family.

• Assist families in understanding diagnoses, treatment plans, community resources, financial assistance programs, educational supports, and healthcare systems.

• Support children and siblings through developmentally appropriate discussions related to illness, hospitalization, loss, grief, and end-of-life experiences.

• Facilitate care coordination among healthcare providers, schools, community agencies, and other support systems to ensure continuity of care.

Knowledge and Skills Requirements

• Knowledge of pediatric development, family systems, grief and bereavement, and psychosocial impacts of chronic and life-limiting illness.

• Strong understanding of family-centered care principles and interdisciplinary team collaboration.

• Ability to build rapport with children of various developmental stages, caregivers, siblings, and extended family members.

• Experience providing crisis intervention, counseling, and emotional support during serious illness and end-of-life situations.

• Excellent communication, documentation, care coordination, and advocacy skills.

• Ability to work independently while maintaining close collaboration with an interdisciplinary pediatric care team.

Experience and Education Requirements

• Current LCSW License valid in the state of Hawai’i

• Minimum 3 years’ direct patient care experience required

• At least 2 years’ experience working with pediatric, hospice, palliative care, behavioral health, medical, or family-centered healthcare populations preferred.

• Experience providing psychosocial support to children, adolescents, and families facing serious illness, disability, grief, or loss preferred.

Working Conditions

• Indoors in patient homes or Hospice Hawai’i homes, nursing, and acute care facilities; may include outdoors in all types of weather

Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours

• Flexible per diem role with the exception of required Monday availability.
