SUMMARY: Provides case management and direct patient care within a team environment. Administers comprehensive pediatric and/or adult physical therapy modalities in the home setting. The qualified therapist assists the physician in evaluating level of function, helps develop and implement and the plan of care (revising it as necessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel and participates in in-service programs. Â Schedule: Monday - Friday, 8 to 430pm Sign on bonus: up to $15,000 Â Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience: - Education:Â B.S., M.S. or D.P.T. degree in Physical Therapy
- Experience: Â Prefer minimum of one year of pediatric experience in Physical Therapy in a hospital and/or home care setting (not including internship).
- Licensure: Current New York State License in Physical Therapy
 Responsibilities - Provide high-quality physical therapy services in patients’ homes, serving both pediatric and adult populations.
- Develop, implement, and update individualized plans of care and discharge plans in collaboration with patients, families, physicians, and the interdisciplinary care team.
- Perform comprehensive assessments and provide therapeutic interventions including gait training, transfers, ADLs, home exercise programs, home safety, and equipment recommendations.
- Monitor patient progress and adjust treatment plans as needed to support optimal outcomes.
- Educate patients, families, caregivers, and team members on disease management, mobility, safety, equipment use, and self-care strategies.
- Supervise and provide clinical direction to PTAs and other support staff to ensure safe, effective care.
- Collaborate with nurses, PNPs, care managers, social workers, therapists, physicians, and other professionals to coordinate comprehensive patient care.
- Maintain accurate and timely clinical documentation in accordance with state, federal, and agency requirements.
- Coordinate referrals and connect patients and families with appropriate community resources.
- Build positive relationships with patients, families, physicians, referral sources, and community partners through professional, respectful communication.
- Participate in patient conferences, quality improvement initiatives, staff education, and orientation of new rehabilitation team members.
- Support agency goals related to quality, productivity, patient satisfaction, cost effectiveness, and regulatory compliance.
- Promote continuous improvement and identify opportunities to enhance patient care and team operations.
- Represent University of Rochester Medicine Home Healthcare professionally and contribute to community outreach and marketing initiatives.
- Perform other duties as assigned to support the needs of the rehabilitation team.
 Salary Range - $67,700 - $115,045 #URM1 Â
