    Description

    As a Patient Support Nurse at CareSimple, you'll be a key liaison between our patients and their clinical partners. You'll build relationships, conduct proactive outreach, respond to patient-generated health-data alerts, and give compassionate guidance that keeps patients engaged and safe between provider visits. You'll also guide patients through onboarding and device setup and manage logistics through our support-ticket system.

    About this role

    This role supports patients across the Pacific and Mountain time zones, with a schedule aligned to West Coast hours.

    What you'll do

    • Patient support and care: guide patients through onboarding and device setup, answer clinical questions, and support engagement and adherence throughout the program.
    • Alert management: monitor incoming health-data alerts, assess severity, apply clinical judgment, and respond through outreach, care coordination, or escalation to the provider team.
    • Technical coordination: provide basic troubleshooting for cellular devices and escalate issues through internal support tickets.
    • Device logistics oversight: coordinate ordering and fulfillment, manage replacements and returns, and keep high-priority orders on track.
    • Documentation: keep clear, timely records across internal systems and, where applicable, the customer's EHR.

    What we're looking for

    • Active RN or LPN license in a Nurse Licensure Compact (NLC) state
    • 5+ years of clinical experience, ideally in telehealth, home health, or primary care
    • Strong communication and interpersonal skills, and comfort working with seniors and underserved populations
    • Technologically confident across EHR, ticketing, and telehealth platforms
    • Authorized to work in the United States without sponsorship
    • Preferred: fluent in Spanish and English; CDE or CDCES certification
