    Patient Engagement Specialist | Brooklyn, NY · $50,000–$60,000/year

In this role, you'll build meaningful relationships with patients, guide them through available resources, and connect them with the right services at the right time.     Key Responsibilities

    Patient Engagement

    • Serve as a primary point of contact for patients, building trust and rapport
    • Proactively reach out to patients to assess needs and ensure they feel supported
    • Educate patients on available programs, services, and resources

    Referrals & Care Coordination

    • Identify patients who would benefit from additional services and facilitate appropriate referrals
    • Coordinate with internal teams and external providers to ensure smooth transitions of care
    • Track referral outcomes and follow up to confirm patients are connected and engaged

    Documentation & Reporting

    • Maintain accurate and up-to-date patient records and interaction logs
    • Track engagement activity and referral data to support reporting and quality improvement
    • Ensure all documentation meets relevant compliance and privacy standards

    Qualifications

    • Experience in a patient-facing, community health, or care coordination role
    • Strong interpersonal and communication skills with a compassionate, patient-first approach
    • Ability to manage multiple cases and priorities simultaneously
    • Familiarity with healthcare systems, referral processes, or social determinants of health is a plus
