Patient Care Manager Opportunity Description
Our client is a well-established Hospice organization. They have a current opening for a Patient Care Manager for their service area in the northern Chicago, IL area. Hospice management experience is ideal but a strong hospice case manager ready to step into a supervisory role will be considered.
Patient Care Manager Job Requirements
Hospice experience required.
Registered Nurse with supervisory experience.
Excellent understanding of Medicare regulations.
Patient Care Manager Job Responsibilities
Manage clinical team.
Ensure compliance with all state / federal regulations.
Train and mentor nurses.
Perform field visits as needed.
