Patient Care Manager for their
San Antonio, TX location. This position is open due to growth! Medicare Home Health supervisory experience as a Registered Nurse is required. This role will oversee the clinical team.
Patient Care Manager Job Requirements - Medicare Home Health supervisory experience.
- Knowledge of Medicare rules and regulations, OASIS, and PDGM.
- Registered Nurse with a Texas license is required.
- CHAP experience a plus.
Patient Care Manager Job Responsibilities - Manage and develop the clinical team.
- Ensure compliance with all state / federal regulations.
- Supervise team and assist in the field if necessary.
