- 1 - 3 years of experience
- Previous home health care or home care experience highly desirable
- Bi-Lingual English/Spanish Required
Responsibilities:
As a Patient Care Liaison, you will:
- Serveastheprimarypointofcontactfornewclientreferralsandservicerequests.
- Monitor intakesystemsandacceptnewcasespromptly,ensuringtimelyassignmenttocarecoordinationteams.
- Verify insuranceeligibility,authorizations,andclientinformationtosupportuninterruptedcare.
- Process referralsandauthorizationsaccuratelywithinestablisheddeadlines.
- Prepare dailyrecoveryreportsandcommunicatecareupdatestoclients.
- Coordinate onboardingactivities,includingwelcomecallsandverificationofdemographic,insurance,andemergencycontactinformation.
- Assign caregiverstonewcasesandconfirmschedules,servicedetails,andclientneeds.
- Facilitate smoothtransitionsbetweenintake,scheduling,andongoingcaremanagementteams.
- Provide exceptionalcustomerservicebyrespondingtoclient,caregiver,andpartnerinquiries.
- Support schedulingactivitiesandmaintainaccuratedocumentationintheelectronicmedicalrecord(EMR)system.
- Collaborate withinternaldepartments,includingcoordination,billing,EVV,andpayerrelations,topreventservicedisruptions.
- Assist witheligibilityaudits,authorizationrenewals,andcontinuityofcareplanning.
- Track admissionandreferraloutcomesandidentifytrendstosupportoperationalimprovements.
- Conduct clientsatisfactionandfollow-upcallstoensureapositivecareexperience.
- Provide administrativeandoperationalsupporttoPatientCareCoordinatorsandClientServiceManagersasneeded.
