    We are seeking a Patient Care Coordinator/Staffing Coordinator to join the Home Health team in our Mission Hills branch!

    Patient Care Coordinator / Staffing Coordinator
    Mission Hills (Northridge Branch)

    Pay Range: 

    LVN: $30-$35 per/hour
    Non-Clinical: $28-$32 per/hour


    What You’ll Do
    • Coordinate and maintain daily schedules for Home Health clinicians and patient visits
    • Manage patient referrals and assign clinicians based on physician orders
    • Process and prepare intake referrals accurately and efficiently
    • Assist with nursing call schedules and staffing coordination
    • Verify Medicare and insurance benefits
    • Communicate with case managers and third-party payors as needed
    • Support interdisciplinary care team meetings
    • Maintain patient confidentiality and uphold HIPAA standards
    • Provide excellent customer service to patients, families, referral sources, and staff
    • Support agency goals, quality improvement initiatives, and daily operations
    • Perform additional duties as assigned
    What We’re Looking For
    • Minimum of 2 years of patient coordination, staffing, scheduling, or healthcare office experience
    • Experience in Home Health, hospice, hospital, or medical office settings preferred
    • Knowledge of Medicare guidelines and Conditions of Participation
    • Strong organizational, multitasking, and communication skills
    • Ability to work independently and collaboratively in a team environment
    • Professional demeanor with strong customer service skills
    • Reliable attendance and flexibility for occasional overtime
    Physical Requirements
    • Primarily sedentary work with occasional walking, standing, lifting, and stair climbing
    • Ability to communicate effectively in English, both verbally and in writing
    • Valid driver’s license, proof of auto insurance, and safe driving record required
