    Job Title: Patient Care Coordinator
    Job Location: Portland, OR
    Job Duration: 3 Months (Possibility of extension)
     
    Shift: Mon- Fri, 11:00 AM to 07:30 PM, Weekend availability required
     
    Job Description:
     
    Required:
    • Oregon & WA RN or compact license active and on file.
    • Must have 1 year of case manager or PCC experience.
    • RN will be needed to work on weekends Sun-Sat w/holidays.
    • EPIC charting and experience with Medicaid/Medicare.
    • All RNs will be expected to float to at least 1 neighboring location.
    Job Responsibilities:
    • Collaborate with the patient, family, and multidisciplinary care team to ensure safe, cost-effective, and efficient inpatient care
    • Evaluate the appropriateness of patient treatments and facilitate any changes in collaboration with the rest of the care team
    • Support, educate, and act as a resource for all nurses and members of the care team
    • Help research, develop, and maintain effective patient care standards and protocols
    • Evaluate patient and family needs for consultation with other healthcare specialists
    • Assess discharge needs and ensure discharges are carried out in a timely manner
    • Ensure that comprehensive information is documented throughout the patient's hospital stay and made available to team members as appropriate
    • Collaborate with all hospital departments to streamline care services and monitor the utilization of resources
    • Prepare written reports and oral presentations of healthcare utilization and cost-effectiveness data as requested
