Pride Health is hiring a Patient Care Coordinator to support our client’s healthcare facility in New Port Richey, FL 34654. This is a 3-month contract assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Pay rate: $14 /hr. On W2.
*Offered pay rate will be based on education, experience, and healthcare credentials.

Responsibilities:

The Care Coordinator supports the clinic by managing patient referrals, coordinating communication with specialists and imaging centers, and ensuring timely processing of referral requests. This role also provides administrative support by handling incoming phone calls and obtaining consult notes to facilitate continuity of patient care.

Key Responsibilities

• Process and manage incoming and outgoing patient referrals.
• Coordinate with specialists and imaging centers to obtain consultation notes and medical records.
• Answer and route incoming phone calls professionally.
• Follow up on pending referrals and documentation.
• Maintain accurate patient records and referral information in the EMR.
• Communicate with providers, patients, and external healthcare offices regarding referral status.
• Ensure referrals are completed in a timely manner.
• Provide general administrative support to the clinical team.

Qualifications:

• High School Diploma, GED or equivalent.
•  Previous referral experience a plus.
