    In our time of Company growth, we are seeking a Patient Care Coordinator - Medical Case Management

    Work Location: Bradenton, Florida.

    As part of this role, you will:

    • Be responsible for day-to-day coordination of HIV patient medical care activities, including the entry of data and tracking of HIV services
    • Identify patients for inclusion in the Program utilizing available metrics
    • Perform comprehensive patient assessments upon entry to service and periodically, including patient and family support systems and needs, utilizing tools as required by funders
    • Develop and implement an individualized patient care plan (that is goal-driven) with a focus on disease management and patient empowerment
    • Assist patients and family with navigation of their health care system
    • Monitor care plans and patients' progress toward goal achievement; revise the plan as necessary, providing education and support as appropriate
    • Provides problem-solving and case management services for patients (medical and social case management) under the supervision and direction of the Infectious Disease Supervisor
    • Formulates problem-solving and case management plans in partnership with team members
    • Monitors patients' progress via the level of engagement in outpatient care and/or substance abuse programs, adherence to treatment plans, successful warm hand-offs to social service resources, and actual ability to access concrete supports
    • Assists with linking and scheduling outpatient visits and follow-ups as needed.     What you need to bring to this role:

    • BA or BS in Public Health, Behavioral Science, nursing, or a similar degree is preferred. A Bachelor's degree in any field is required.
    • RN or LPN with a minimum of 1 year of experience working with Hep C and HIV patients preferred
    • RN or LPN with two years of experience performing similar tasks preferred
    • Experience working with patients with multiple diagnoses of HIV, HCV, mental illness, and substance abuse as well as diabetes and heart disease
    • Strong interpersonal communication skills, excellent public speaking abilities, strong written and verbal English skills
    • Experience working in a healthcare environment with interdisciplinary teams
    • Must have reliable transportation with the ability to conduct fieldwork

    Under the direct supervision of the Infectious Disease Supervisor and the Director of Care Coordination, the Patient Care Coordinator - Medical Case Management will support HIV grant programs to ensure routine screening while identifying newly diagnosed individuals or those out of care for linkage to HIV treatment and support services.

    The primary responsibility of the Patient Care Coordinator is to manage care for patients with HIV. The Care Coordinator's role is to work in conjunction with medical providers and social support to provide case management, education, outreach, and logistical support to assist patients in successful linkage to care, program enrollment and eligibility assistance/maintenance, retention in care, and achieving optimal health outcomes. Additionally, the individual is responsible for, with guidance from the Infectious Disease Care Coordinator Supervisor, patient education on the processes for obtaining and adhering to medication. The Care Coordinator will be the point of contact for all assigned patients, ensuring continuity of care and services through the referral and treatment process.
