Connected Health Care is partnering with a leading healthcare organization to recruit a Patient Care Coordinator / Liaison Marketer for a full-time, field-based opportunity in Meridian, Idaho.
As the face of the organization's Home Health and Hospice services, you will develop referral relationships, grow business throughout the Meridian area, and connect patients with appropriate care. This position works closely with physicians, hospitals, discharge planners, case managers, social workers, skilled nursing facilities, assisted living facilities, and community partners.
Job Description
Develop and maintain relationships with physicians, hospitals, case managers, discharge planners, SNFs, ALFs, and other referral sources.
Visit hospitals, healthcare facilities, physician offices, and community organizations throughout the Boise area.
Promote Home Health and Hospice services and develop new referral opportunities.
Manage and track referrals from initial contact through admission.
Communicate with internal and external teams regarding referral status and admission needs.
Assist with admissions, transfers, discharges, and insurance authorization processes.
Maintain knowledge of Medicare, Medicaid, private insurance, and other healthcare payors.
Participate in networking, community outreach, and healthcare marketing activities.
Maintain accurate referral, census, and patient information.
Provide exceptional customer service to patients, families, and healthcare partners.
Maintain patient confidentiality and follow applicable healthcare regulations and organizational policies.
Education and Requirements
Healthcare sales and marketing experience is required.
Minimum 1 year of successful business development or marketing experience in healthcare, skilled nursing, assisted living, Home Health, Hospice, or a related setting.
Associate Degree in Business, Marketing, Healthcare Administration, or a related field preferred but not required.
Strong communication, interpersonal, networking, and relationship-building skills.
Working knowledge of Medicare and Medicaid eligibility preferred.
Clinical healthcare knowledge is a plus.
Valid driver's license, auto liability insurance, and reliable transportation required.
Must be able to obtain required Reptrax/VendorMate credentials for hospital access.
Highly motivated, energetic, organized, and comfortable working independently in a field-based position.
