Description

As a patient care coordinator, you will work collaboratively with our clinical staff as well as other department staff to provide high-quality care for our patients. This position is full-time, working Monday-Friday 1:30pm-10pm. $19-22/hour. Duties: Intake and discharge of patients for provider appointments and ancillary services; Use a variety of communication tools and systems to handle patient interactions and work collaboratively with the rest of the clinical department; Maintain complete, timely, and accurate documentation for each patient and continually look for opportunities to improve processes and workflow; Complete other assigned and clinical department tasks. Key Skills: Create a positive and welcoming patient experience; Ability to prioritize patient requests; Ensure accuracy of patient data; Strong written and verbal communication; Ability to multitask; Ability to de-escalate conflict situations.
