Your Role as a RN: We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available as part-time, with flexible scheduling. Care locations would be primarily in Eagle Mountain & Saratoga Springs.
You'll work closely with an interdisciplinary team to:
Conduct admission and OASIS assessments Perform head-to-toe and wound evaluations Create and implement individualized plans of care Provide follow-up skilled nursing visits Educate patients and caregivers Coordinate care with MDs, CNAs, PT/OT, and family members Attend bi-weekly team meetings and contribute to care planning
Pay and Benefits

Qualifications:
Current Utah RN license Reliable transportation Excellent communication and interpersonal skills
Preferred Qualifications:
