By bringing care directly into the patients home you will help to identify and address the underlying factors contributing to repeated EMS and hospital utilization. Essential Duties
Administer and direct basic and advanced life support emergency medical care. Provide guidance and support to basic life support providers.
Conduct in-home medical assessments of identified high-utilizing and medically complex patients. Identify medical behavioral environmental and social factors contributing to recurrent EMS and hospital utilization.
Develop individualized care plans in collaboration with patients caregivers healthcare providers social service agencies and other community partners.
Provide appropriate medical and social-service referrals and facilitate connections to available community resources. Help patients navigate available healthcare and social-service systems.
Coordinate care with primary care providers specialists hospitals EMS agencies behavioral health providers and social service organizations.
Follow up with patients to promote treatment adherence appropriate use of healthcare resources and improved health outcomes.
Maintain accurate and timely clinical documentation and communicate relevant findings to appropriate healthcare partners.
Participate in ongoing evaluation of program effectiveness including tracking patient outcomes EMS utilization hospital utilization and other performance measures.
Work as part of an interdisciplinary team to address both the clinical and social determinants contributing to recurrent healthcare utilization.
Check inventory and maintain equipment vehicle and medical supplies. Complete inspections to verify vehicles tools and equipment are properly maintained.
Participate in training drills and other related activities.
Ensure all work is complete and timely to support department goals. Evaluate feedback and procedures to improve efficiency.

Class C non-commercial drivers license
Cardiac Rescue Technician Paramedic certification Maryland or National Registry NREMT Paramedic

Community Paramedic Credential CP-C by IBSC or must obtain within one year of employment
Required certification training for Common Passenger Vehicle Technical Rescuer -Technician or Rescue Technician site operations vehicle and machinery will be provided upon hire. Continued employment is contingent upon successfully completing and passing the certification.
Required certification training for Emergency Vehicle Operator will be provided upon hire. Continued employment is contingent upon successfully completing and passing the certification.
Required certification training for HazMat AwarenessOperations will be provided upon hire. Continued employment is contingent upon successfully completing and passing the certification.
Additional Information
Must be able to safely perform the duties of the position without posing a threat to the health and safety of themselves and others.
Will be required to work outside of standard hours.
Facial hair must be maintained to properly wear job-specific safety equipment and pass SCBA fit test throughout employment.
Requires criminal background check as condition of employment.
Requires medical clearance consistent with NFPA 1582.
