    What You Need to Know:

    Overview

    AccentCare is seeking a palliative care visit physician to provide comprehensive medical oversight and direct patient care within the Memorial Healthcare system. This physician will conduct patient rounds according to the required schedule, coordinate physician coverage, participate in discharge planning, and provide trusted clinical leadership and education to care team members. If you are looking to play a vital role in creating a supportive environment where patients experience comfort and dignity, families receive guidance and reassurance, and care teams work together, then this is the position for you.

     

    If you meet these qualifications, we would love to meet you: 

    • Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients
    • Maintains active DEA license
    • (Required) Palliative care fellowship
    • Working knowledge of hospice and palliative care, including pain and symptom management.
    • Familiarity with hospice regulations and Medicare requirements.
    • Strong clinical judgment and decision-making skills.
    • Excellent communication skills for interaction with patients, families, and providers.
    • Ability to manage complex clinical and ethical situations in end-of-life care.
    • (Preferred) Hospice or palliative care visit experience

    Job Responsibilities:

    • Develop and maintain Plan of Care for each patient, including discharge planning discussions with care team, patients, and families
    • Participate in prognosis evaluation and certification/recertification
    • Sign death certificates as needed
    • Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team
    • Ensure proper documentation of eligibility for inpatient care
    • Provide education and training to interdisciplinary team and new staff
