JOB SUMMARY:

The Palliative Care Social Worker applies principles of social work, pediatric palliative care, family systems and behavioral health practice to provide longitudinal psychosocial assessment, counseling, crisis intervention, care planning, advocacy and case management for children with serious illnesses and their families. As an embedded member of the interdisciplinary Palliative Care team, this role supports inpatient, outpatient/telemedicine, and perinatal palliative care setting, partnering with medical teams and community agencies to address coping, communication, resource needs, advance care planning, grief and bereavement, sibling/family support, health equity and barriers to care. The social worker develops and documents care plans, provides family-centered and trauma informed interventions, supports transitions across settings, participates in team meetings/QI/education, facilitates groups, and may provide staff debriefing or consultation after distressing events. Duties are assigned by Social Work and Palliative Care leadership. The Palliative Care Social Worker will follow the patient across the care continuum (outpatient, inpatient and home). The Palliative Care Social Worker will conduct home visits as part of comprehensive services.

MINIMUM QUALIFICATIONS:

Master's Degree in Social Work

1 Year of Experience

CA Driver's License

Proof of Auto Insurance

Relevant community-based palliative care experience, healthcare experience, and/or mental health experience

Experience in trauma, bereavement, chronic illness, catastrophic diagnosis, child protection, and bioethical issues

California driver's license and proof of insurance will be required for home visits.

Demonstrated commitment to respecting differences and contributing to an inclusive working and learning environment

PREFERRED QUALIFICATIONS:

3 Years of Experience

LCSW - Licensed Clinical Social Worker

3-5 years of experience as a social worker in a hospital setting which includes providing education and consultation to interdisciplinary staff and working with community organizations

Knowledge of the principals of growth and development over the life span

Knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, crisis intervention, and de-escalation

Familiar with NASW standards of Palliative and End-of-Life Care

The current salary range for this position is $41.49 to $57.05
