We are seeking a compassionate and highly organized Registered Nurse to provide individualized in-home Palliative Care to patients living with serious, chronic, or life-limiting conditions throughout our local communities.

This is a unique nursing opportunity for an RN who enjoys autonomy, relationship-based care, and working at the highest level of their scope of practice. Rather than following a traditional hospital-floor schedule, our Palliative Care RNs have flexibility in organizing their day based on patient needs, visit frequency, location, and documentation requirements.

What Makes This Position Unique

• Flexible scheduling: Full-time employees may work four 10-hour shifts or optional five 8-hour shifts

• Our current 4 - 10-hour rotation provides a 4-day weekend every other week

• RNs have significant autonomy in organizing their daily patient schedule

• Patient assignments are coordinated geographically whenever possible to minimize unnecessary travel

• Nurses based in the Walla Walla/Waitsburg area may primarily support Waitsburg-area patients while also seeing patients in Dayton as needed

• Office/workspace is available in Dayton and Waitsburg for scheduling, documentation, and administrative work

• Mileage is reimbursed for work-related travel after the initial commute to your designated workspace

• Dedicated documentation time can be incorporated into your schedule throughout the day

• Opportunity to develop meaningful, long-term relationships with patients and families

• Work within an Interdisciplinary Team of ARNP, RNs, MSW and CHW
  
  

What a Typical Day May Look Like

No two days are exactly the same. Visit frequency is based on each patient's individual needs and may range from several visits in one week during an intake or period of increased need to routine visits a few times per month.

Our nurses manage their own schedules within the needs of the program, allowing them to organize patient visits geographically and efficiently. Because patient needs can change, flexibility, strong organization, and time-management skills are important.

Qualifications

• Active Washington State RN license or active Multi-State RN License

• BLS certification every two years — we can assist with obtaining certification

• ACLS a plus

• Strong organization and time-management skills

• Ability to work independently while collaborating closely with an interdisciplinary care team

• Compassionate communication skills and a commitment to patient- and family-centered care
  
  

Responsibilities

• Provide comprehensive, patient-centered nursing assessments and ongoing symptom management

• Coordinate care for patients with serious, chronic, or life-limiting conditions

• Develop and update individualized plans of care in collaboration with providers and the interdisciplinary team

• Provide education, guidance, and emotional support to patients and their families

• Coordinate referrals, services, and community resources

• Respond to changing patient needs and adjust visit frequency as appropriate

• Maintain timely and accurate documentation within the electronic health record

• Communicate closely with providers, patients, families, and members of the care team to support continuity of care
